CareCredit: When unexpected illness/injury strikes, unexpected expense strikes as well. We understand this and therefore accept payment through the CareCredit program. It takes just 5 minutes to complete an application and will allow you to break down your payment into 6 monthly installments. You can apply yourself online at CareCredit.com/apply.
Insurance Options
We accept and are in network with most health insurance options. Please check with your insurance company to make sure that we are an authorized provider. Please call our office (801-262-3600) if you have any questions.
If your insurance requires a referral or pre-authorization, please obtain one from your primary care physician and bring this with you to your appointment.
