Appointments
It is our policy to see patients as close to the scheduled appointment time as possible and in the order scheduled. If you arrive more than 15 minutes late, you may be asked to reschedule for a different day. New patients should arrive 10-15 minutes early to complete required forms.
It is the policy of our office to charge $45.00 for missed appointments if the appointment isn’t cancelled more than 24 business hours in advance. Missed surgical appointments are charged $150.00.
Insurance and Billing
We accept and are in network with most health insurance companies. Please check with your insurance company to make sure that we are an authorized provider. Please call our office 801-262-3600 if you have any questions.
If your insurance requires a referral or pre-authorization, please obtain one from your primary care physician and bring this with you to your appointment.

Payment Options
We accept payment by cash, check and credit card. Co-pays and deductibles are due at time of service. If you are unable to pay at the time of service, your appointment will need to be rescheduled.
